Southwestern Public Health is committed to getting COVID-19 vaccine to seniors living in our community.

If you, or someone you know, was born in 1946 or earlier and has not been able to book an appointment for a vaccine, please provide the following:

Which Southwestern Public Health region do you live in?
What is your preferred clinic location?
If necessary, could you get to your preferred location with 30 minutes of notice?

Thank you very much for providing us with your information.

You will be phoned by a volunteer or staff member from Southwestern Public Health with the date, time, and location of your appointment. Please have your health card number handy.

Important to know:

  • If you have submitted the name of someone else, please ensure they are expecting our call and that they want to receive the COVID-19 vaccine.
  • Please do not complete this form for someone who already has an appointment booked.
  • Everyone being vaccinated must show proof of identity when they arrive at the clinic or they will be turned away.
  • This is NOT a registration page or a waiting list. Please continue to try to book an appointment for yourself or for your family member or patient/client.
  • These doses may be end of day doses, or those leftover for someone who did not show up for their appointment. That is why we have asked if you can arrive within 30 minutes. If you are unable to make it in that time frame, don’t worry – we will still each out to you to help you get booked.
